Now bare in mind, my swing just got ripped apart. But looks like you are pretty far away from the ball and your feet are closed to the target line. Or perhaps the camera is not on the target line. Do you hit a push hook? I bet your good shots are neat draws, but your bad ones are dying quail snappers. How tall are you, BTW- club looks kind of long. I'm not sure I really know what I'm talking about though.

Club is nicely swung, stays on plane rather well.... but your grip looks rather odd, hard to tell from this angle... Looks like you are way too much in the palms (especially with the trail hand), restricted wrists... which is why you tend to fade. Can you post some pics of your grip?
 
I suggestion that I have is not to block or stop your swing. SWING all the way around (finish ala Ernie Els with the club wrapped around you) This will give you more distance and will also help release the club. To many golfers are decelerating the club rather than swinging thru the ball. Just my 2 cents
