Deep divots, thin & fat shots

I can't watch the videos at work but my guess is a lack of axis tilt or to be honest, sometimes in softer conditions divots will be deeper.
 
I took a look at the videos and I like the overall motion of your swing. I am sure the manzella staff can do a better job than I can but I will give it my shot. Your swing plane on the backswing is above the plane. You have to loop the club back down to get it back to the original shaft plane. Usually this would require more hip drive which as jim said would require more axis tilt. In your case your leg and and hip slide action does not match up with your excellent hip rotation .

As I see it you could change your backswing plane and get the butt end of the club pointing more towards the ball or you could work on a stronger lower body motion on the start of the downswing.

You're right about the backswing, the butt end of the club points too much inside the target line. Do you think fixing this will change my downswing?

If you look at the dynamics of your swing legs etc. hips ,Knees are not keeping up with rest of the swing. You could keep the plane alone and focus on moving your lower body faster which would increase the hipslide which would move your low point more forward of the ball. Bottom line let the lower body lead the swing all the way thru with a good weight transfer.

Or you could work on your swing plane and get the backswing more on plane and see how that affects the swing. Its a matter of choices and how succesfull you have been in repeating consistently a onplane downswing. If you have a laser system play around a bit with this and see if it makes the downswing easier.

Other feedback by Brian or his staff would probably be better advise than what I have offered. Looking at a swing on a camera and seeing things that are outside the orthodox and then trying to determine the root cause takes a lot of experience teaching like Brian and his staff have.

I have actually stumbled upon a solution for my problem.

I've always tried to get my hands in front of my body and in front of the ball (to create the most delofting I can..). My right elbow went in front of my right hip.

I tried to create a more shallow approach to the ball by sliding my right elbow down the right side of my body instead of it going right in front of my body. It's working very well, my divots are now much thinner, and I can even hit my wedges pure, which I never really have. I checked what this looked on video. I had less trigger delay but I was getting to a good impact position. This is going to help my iron game alot.
