You can have throwaway with a flat left wrist, but you lose compression and clubface alignment becomes erratic. Read 7-19. Sustain the lag!
 
Try perfecting your grip. Im sure its not under the muscle pad in the left hand. Also check your right hand. Make sure the "V" points to your right shoulder and the lifeline on your right hand (between the 2 muscle pads) sits on the left thumb. From there if you still get a flat wrist, check your body motion. If your middle dosent work properly, something has to take the load...in this case...maybe that left wrist!!!!!!

I believe acc#4 would be more of a leekage problem as the pivot is doing all the work to apply radius power.
