Hands in front of ball at address = slice/shank. Why?

In recent years I have struggled with a bad slice/shank. Since reading on this forum I have changed my hand position at address. I now start with my hands behind the ball and with the twist away it has really helped. Now, even when I am playing well, I can still occasionally hit a very ugly shot that starts way right and then goes even further right and never gets high into the air. I was doing it all the time until I came to this forum.

It still shows up from time to time and I don't know why. If I was to start with my hands in front of the ball I bet I would be hitting those kind of shots over and over within 10 minutes. Without seeing my swing can anyone describe what I may be doing that causes such a shot. I know I come from the inside with the face open but I don't know why or what I do differently than when I hit it well. I initially was using the twist away after first coming to this forum but I don't consciously do that anymore. My left wrist is flat or even bowed at the top, club face facing a little to the sky (shut?). My swing is flattish.

I know my question is kinda vague but Brian Manzella and the members here have helped me tons without me asking a single question or knowing any of my problems. I figured I'd ask a few straight questions and see what kinda magic happens. ;) Thanks for the help.

Such a ball flight can occur when you impact the ball with the (slightly open) leading edge of the club face on the way down.
Hands too far in front of the club head at impact is the usual culprit.
