I would really appreciate some help

I've been dealing with this issue for a few years now:

On my backswing, everything looks great till I get to about the 3/4 point then I start to lose my axis tilt till i'm almost completely straight at the top. This seems to start a chain reaction where my upper body starts to lead the downswing, my arms get left behind and at impact my head is slightly ahead of the ball. I obviously then have a great deal of trouble squaring up the clubface and while I hit the ball solid, it often is a push with a slight fade. It is also hard for me to straighten the left leg through impact.
Every teacher I've been to just tells me to shorten the backswing but I have a hard time with that. When I swing in slow motion I have no problem reaching the top without losing the axis tilt so I don't think it's a flexibility issue. I have tried to allow my hips to turn more on the backswing but that doesn't help either. I've tried to let my arms swing more around me instead of up but still no success. I have used the techniques in NSA and while that helps square up the clubface, it feels more like a band aid then a solution. My impact position is clearly ahead of where is should be and I feel that is why the face is open.

I still manage to get around with a 5 handicap typically but aiming a bit left knowing that the ball will go a bit right but I know I could improve my ball striking if I could just figure this out.

Any help would be greatly appreciated.

you need to make a much better backswing pivot; study the "perfect pivot" article #1. IT's the same pivot described in NSA. If you do it correctly, you won't have this issue.
 
believe me I have over and over. Problem is I can't feel when the breakdown happens during the swing. I've rehearsed in front of a mirror and everything looks good but put a video on me and you could puke.
 
I've fought the same problem, for me it was because I needed to shorten my backswing, and I struggled with doing that too (as you said you did). The problem was the timing and the sequencing was so off because my "natural" swing was so much longer. What I worked on was 3/4 wedges at the range to get a feel for the new timing and sequencing that a 3/4 swing required. Using the wedge first makes it easier. There was a lot to change then in terms of timing, like cocking the wrists earlier in the backswing and the transition was the hardest, but it helped me shorten my backswing and prevent turning so far back in the backswing that I started to reverse pivot. I am just the opposite. My swing in the mirror goes almost to parallel, but on video doesn't get close, it looks 3/4, subsequently I get quick with my right arm getting more "on top" instead of working "underneath"

Same here. I just did some new video yesterday and my bacswing with a 6 iron is only 2/3!!!! I am hitting it crisp and straight but only about 160-165 yards. Maybe I should lengthen it? I don't know, I just know that it looks too damn short!
