You have a closed clubface at the top. Usually the way people play with that and don't hook it all the time is by tilting their back away from the target during the downswing ala David Duval. Which I'm guessing is what you used to do before the back surgeries, and what may have contributed to the back surgeries. So since that option is out you should probably stop pointing the clubface skyward at the top. Maybe weaken the grip, bend the left wrist, take it back more upright instead of letting the clubhead get back behind your body, or find some other way to hang the toe more.
