Thoughts on hitting on the toe.

I have been following TGM for over a year now, when I first started to apply some of the teachings I started to play the best golf of my life ball striking wise. I have started to hit everything on the toe irons go straight and sometimes hook but hit on the toe. Any type of wood is just a big smother hook I cannot stop hitting on the toe and cannot figure out what is causing it I have a couple of Brians videos they are great and have helped me alot. My only swing thought is to bend my right wrist straight back and hold it until after impact is there some other thought I can focus on that might help cure this.
 
Are you having clean shot? Or are you hitting a bit fat. Are you hitting the top part (Club face) of the driver.
If I am correct the right wrist bend is because of the force and motion, it should be a totally free moving hinge.
