What causes a high hook???

I was playing with a friend this weekend, who was consistently hitting very high hooks? I was baffled as to what type of swing would cause that. I know that a low duck hook is typically caused by a flat(under the plane) swing with a closing club face. Typically when people hit a high shot, it's because they are coming in too steep, however, this usually results in a slice. It was almost like he was popping the ball up(ball hitting high on the club face), but the ball would start straight or right, and then turn left. Any thoughts?
 
I've seen lots of people do this, when they tee the ball high and swing up and out with driver. If it's with the driver, make sure he knows you dont hit up on the driver, no matter what some mainstream instructors would have you believe
