Underplane/Flying wedge help

Ok. here is my problem and what I think I may be doing, but not sure of a correct fix.
misses and what has turned to be a consistent shot lately is a ball that either starts straight or even a bit left of target and goes further left. my divots go a bit to the right, which tells me i am going in to out on my path. so i guess it's a face issue as well. I used to feel i am underplane, but I also feel that coming into the ball my right elbow/forearm is higher or on top of the left. This may be the cause of the face closing early? I'm not quite sure of the flying wedge concept, but i guess this is not correct. So my thinking is getting the right forearm on line with the shaft coming to impact. Should this help delay the closing of the face? Thanks for any clarification or help.
 
I believe you are correct. I think if it starts left and curves left it's both face and path. Would the "yellow brick road" maybe help you? Do you have NHA?
