This is one week after coming back from a 6 month layoff due to herniated disk surgery. I would like your general thoughts and opinions..........

That's definitely more of a stack and tilt action than reverse pivot.

Reverse pivot is when you pivot on your left leg on the backswing, and on the right leg on the downswing.
 
Does your left foot need to be that flared open? If so, why? Sorry that I don't have any constructive criticism to go with my question.
 
I would imagine that exessively flaring the left foot would help him rotate more on the downswing from the extremely left leaning position at the top of his backswing. With average flexibility and a history of back problems i.e. a herniated disc, it would be difficult to make a full rotation without getting "stuck" in some way. A useful technique in this sense, but not very powerful.
