Cause of hacker swing...

What's the cause of the hacker swing, specifically, where at impact or close to it that the right side is convex (or "stretched") and the left side is concave (or "dented") as opposed to the other way around?
For the most part, I've gotten rid of that, but it still comes back to haunt me every now and then. However, if there was something I could do or a way stop it permanently, that would be great.
Do you think I did that in an attempt to correct for a slice?

At the core, an open clubface USUALLY. If the face is open most people will at some point try to save the shot by coming a bit OTT and this forces the right shoulder above plane and does what you say.
