Steering.

Trying not to Hook it.

Hitting UP on it.

Too flat going back.

Too inside-out.

No pivot rotation through impact.

No left arm rotation through impact.

Reverse-Pivot.

Boy, that is the same problem I have. Out of the eight things that Brian named that can cause this problem I think I have five of them. No wonder I have no follow through.
