hands behind rt shoulder

Swing on a steeper plane. i.e. trace a straight plane line (flashlight drills).....

You could be rolling the clubface open too much too...that's a common reason why people go sharply to the inside of the plane line on the takeaway.
 
Jerry, you are probably sucking in the club too far inside. Your arms maybe making a sharp turn instead of the shoulders.
Let the bigger muscles start the club back.

On take away, with your right hand, simply draw the clubhead in a line straight back from the ball until it automatically arcs inward towards you. From there on in, simply bend your right arm and allow the club to come up until your hands are both at shoulder height, left wrist c0cked and right wrist bent.

Provided you got the first part right you will have turned your upper body back and away from your target as far as it NEEDS to go and your hands will not be behind your shoulders..
