hitting the ball too low, losing distance

I got the golfing machine book several months ago and it correlates well with what my GolfTEC instruction teaches.... so I have been practicing the flat left wrist, clubhead lag (forward swing bottom) and correct plane in my basement for a couple of months - I finally get out to the driving range and seem to have a significant problem hitting the ball too low- it takes off well with lots of spin but stays low and does not achieve a normal distance - with Ben Doyle's admonition you can't have too much clubhead lag and Bobby Clampett forward swing bottom of 4 inches, I was suprised this could be an issue - any advise? How do I know the correct downward angle of impact?
 
Shaft needs to have less forward lean.

First thing to check probably is axis tilt.

Ok, Thanks. will work on that next.... How will I know when I have the right amount of forward lean - I guess it would be when the ball is going the furthest, but I used to hit the 7 iron 150 - 155 when I "flipped" it, not hitting it as far now - it goes much lower....
