Yeah, something is definitely wrong with that picture. The ball is far too low to the ground and close to the clubface, given the distance between the start of the divot and the current position of the clubhead. Taht is, by now the ball should be long gone and much higher.
I think this is video of a mis-hit, and therefore probably not a good one to diagnose for shaft lean. Personally, I have rarely ever seen any but the very best players who were good enough to have the "problem" of too much shaft lean. At least from this one shot, I'm not sure that this golfer should be worrying about shaft lean.
