Left shoulder over right foot?

What percentage of right handed golfers can actually get into this position using a hands controlled pivot and a proper pivot action?

All of them, or not many at all, depending on your definition of 'proper pivot' and the hip vs. shoulder and left foot motion
 
Within normal stance widths, if you do this, you must make a huge lateral shift to get back to a position to be able to hit the ball effectively. Viewed from the front of the player, if you properly preserve the angle of the right leg, this is impossible. Ballard teaches this - laterally shift the right hip over the right foot, rotate around the right leg, laterally shift the left hip over the left foot, rotate around the left leg. Notwithstanding that this method won 2 consecutive US Opens, a better way is to rotate into the inside of the right foot in the backswing. A pressure shift rather than a weight shift.
 
MJ, I would agree that the casual golfer would have difficulty coordinating that degree of lateral movement. They don't have the luxury of playing golf everyday of their life, as Curtis Strange did.
I also wouldn't be as concerned about an "A" To "B" objective in this part of the swing. By "A" To "B", I mean every golfer, every body type must be in a specific position;ie. L. shoulder over right foot. I think I would be more concerned with them feeling that they load the club and not put it in a position up top that would require huge compensations. The Physician's Code," First Do No Harm"!
