Looks like you hands are too far out when they get around mid-body. If you freeze the video, the club is steep through your right shoulder. You need it to cut you in half more around middle of your bicept. Your hands are out and working to in. You need to get them more "in" by keeping your left arm in and dropping your hands more down.
Try this movement with no club. Take your left hand to the top, stay turned and drop it down and slap your right thigh. This is the opposite of what you do. Your hands stay high and work out. Next get your club and a mirror. set up so you can watch yourself from down the line. Take it to the top and practice dropping your hands so you can see how the shaft looks like it cuts your right bicept in half. Practice lowering it down while your shoulders stay turned. You can move forward but keep the shoulders turned and lower the shaft.
