shots in the dark, dont come from the inside out so much, rotate left forearm faster. Just a couple of guesses. If it crosses between a block and snap hook then it could be getting stuck on the downswing, again, just a guess

You're getting to the inside of the ball with an open face, so you just need some face closing through the impact interval.

Assuming a decent setup, either speed up your arms or slow down your hips it is likely a timing problem.
