Probables:
1). Your "fade that starts straight and fades away" may be caused by you coming a little too inside to out in an attempt to steer the ball to the target.
2). Your "start right fade that doesn't" may be caused by an outside to in swing path as you try to steer the ball to the right.
3). "Pulls", an extention of 2)., are your straight away shots but with a little outside to in contact, possibly due to the ball being positioned a little too far forward.
If you are an habitual fader then you DO need to allow for this and simply adjust your target line accordingly. Swing down your adjusted target line and NOT at the target itself.
I play with a couple of guys who do these things regularly and this is what I see them doing - they are, curiously enough, both lefties.
